The four multiplayer maps are Skyjacked (a re-imagining of Call of Duty: Black Ops II’s Hijacked), Splash, Gauntlet, and Rise. The Zombies map is called Der Eisendrache; it picks up where The Giant left off.
Awakening is included with Black Ops III’s $ 50 DLC pass, which also comes with three further expansions to launch in the future. Alternatively, you can buy Awakening alone for $ 15.

And for Zombies fans…your commitment to us pays off all season long, starting with Der Eisendrache.”
Awakening launches first on PS4 as part of a timed-exclusivity agreement between Sony and Activision. The expansion will come to Xbox One and PC sometime later.
